Records and Requests
Requests for a copy of your medical records are usually made in writing with proof of identity, and are handled by the hospital's records department.
Ask for the response period when you submit the request, and keep a copy of everything you send.

Where a concern goes
Raise it first with the person directly involved, or with the department or patient services desk. If that does not resolve it, put it in writing and ask for an acknowledgement and a named person to handle it. Ask at any stage what internal and external routes exist.
